1. Passport gốc / Passport original hard copy

2. Hai tấm hình 2x3 / Two photos 2x3

3. Xác nhận tạm trú tại địa phương photo / Photo of record of temporary residence at local address

4. Mẫu NA6 (công ty đóng dấu ký tên) / Form NA6 signed and stamped by company: N6

5. Mẫu NA8 (đơn đương ký tên) / Form NA8 signed by applicant: N8

6. Giấy phép kinh doanh sao y công chứng / Business license notarized copy

7. Giấy phép lao động bản gốc / Work permit original hard copy

8. Thẻ tạm trú cũ bản gốc. Hoặc visa thương mại còn hạn nộp kèm công văn nhập cảnh / The old TRC original hard copy. Or valid business visa and VOA.

Làm cho vợ/chồng / Apply for spouse:

1. Giấy đăng ký kết hôn đã hợp pháp hoá lãnh sự, dịch công chứng tiếng Việt / Marriage certificate with consular notarization, translated into Vietnamese

2. Passport gốc của vợ/chồng / Passport of spouse original hard copy

3. Hai tấm hình 2x3 của vợ/chồng / Two photos 2x3 of spouse

4. Xác nhận tạm trú tại địa phương photo / Photo of record of temporary residence at local address

Làm cho con / Apply for children:

1. Giấy khai sinh của con đã hợp pháp hoá lãnh sự, dịch công chứng tiếng Việt / Birth certificate with consular notarization, translated into Vietnamese

2. Passport gốc của con / Passport of children original hard copy

3. Hai tấm hình 2x3 của con / Two photos 2x3 of children

3. Xác nhận tạm trú tại địa phương photo / Photo of record of temporary residence at local address
